Kai is an enormous man, overshadowing almost everyone else in Soul Society in terms of height, as well as possessing a body so muscular it can create enough shade to cover a small vehicle. Kai has dark skin that is rough to the touch and littered with scars all over the front side, possessing no scars on his back. Kai has strong facial features, possessing very masculine traits such as a broad chin with a cleft, a large and prominent brow, and a somewhat bent nose. His eyes are a tad bit close together, but not awkwardly so. On his brow, Kai has three scars that span out in equal distance from each other, all roughly the same length. The scars all come close together at the center of his eye brows, and fan out in a somewhat crooked way in opposite directions. He is completely bald with a single red warrior stripe down the center of his head.

Kai wears the standard shinigami uniform with only minor alterations. He does not wear the customary white undershirt, and the sleeves on the black overshirt are completely ripped off, leaving jagged marks at his shoulder. His zanpakuto is sheathed at his left side, its long sheath creating a tail behind him. The final thing Kai wears, though is not technically a part of his uniform, is a serious face. Unless in battle, or contemplating a battle, or laughing at someone's misfortune, Kai looks as if he is being bored and bothered at all times.
